Injury / Surgery

It is the sole responsibility of the parent/guardian to notify the school nurse as soon as possible of ANY physical injury or changes in a student's health status.

Please review if your child is returning to school with any of the following:

If any of these physically limit a student from participating in any sports or education, then a doctor's note needs to be obtained and provided to your designated school nurse.

On the other hand, if a child has had a recent surgery or concussion, a clearance note from their physician is required to return to normal activity without restrictions (which includes returning to sports/extracurricular activities).
