Injury, Surgery or Concussion
If your child is returning to school with any:
- physical injury,
- status post any surgery,
- any other issue that would physically limit a student to participate in any sports or education,
then a doctor's note needs to be obtained and provided to your designated school nurse.
It is the sole responsibility of the parent/guardian to notify the school nurse immediately of ANY physical injury or changes in a student's health status.
Excusal Notes from Parents
Other things to be taken into consideration:
- A parent may provide a written excusal from physical education for ONLY 1-2 days, after that a doctor's note is required
- A doctor's note is required for the use of crutches in school
- A doctor's note is required for casts, arm slings and use of any splints
- A doctor's note is required for Return To Play (RTP) in sports after a suspected or diagnosed concussion [please view our Concussion info]
A parent may provide a written excusal from physical education for ONLY 1-2 days, otherwise a doctor's note is required.
Physician Notes
If you need to provide a doctor's note for a student's injury or status post surgery, please make sure it includes the following:
- Nature of the injury,
- any limitations or restriction(s),
- the date(s) the student should be excused from any physical activity and/or limitations INCLUDING physical education and recess
All devices such as crutches, canes, walking boots, etc, require a doctor's order.
